i wanted to start playing witcher 3 for the first time about a month ago, but i can't: i get through the intro and to the main menu just fine, but whenever i try starting a new game it always crashes to desktop right after the first cutscene.
i've spent hours trying every solution that i could find on this forum, the steam forum, gog's support page and other sites from google searches, but nothing made any difference. some of the things i've tried:
- verifying/repairing the game
- reinstalling/downloading the game
- reinstalling latest gpu driver, after doing a clean uninstall
- reinstalling a gpu driver from before the game's release, after doing a clean uninstall
- reinstalling directX
- all possible combinations of fullscreen/windowed/borderless and 30fps/60fps/unlimited from the ingame menu
- several different combination of other ingame graphics settings, like setting everything to the lowest possible
...and probably a few other things that i don't remember now.
i had posted about the issue in this thread, where someone from cd-project let us know they were "currently investigating this issue". but one month and two patches later, the issue still persists and i haven't found any updates from cd-project.
my system:
GPU: Sapphire Radeon HD 7870 XT 2GB
CPU: Intel Core i5-3470 3.2GHz Quad-Core
Motherboard: Asus P8B75-M LX PLUS Micro ATX LGA1155
Memory: G.Skill Ripjaws X Series 8GB (2 x 4GB) DDR3-1600
i've installed via gog galaxy, i'm not using any of the free dlc (i haven't 'bought' them yet) and i'm not using any mods.
The only thing I can think of is that you need to allow the game access through Windows UAC. Best bet is to completely disable UAC, but that's not advisable to do if you have lot's of things installed. Plus, you'll lose all of the protection that comes with it. The other option is to ensure you are logged on as an administrator, have installed the game as an administrator, and run the game by tweaking the witcher.exe 's compatibility to "Run as administrator".
I think you can set other things aside. At the exact point that Windows is supposed to give you control of the game, it sounds like something is hanging your system. That smacks of Windows "security features".
Try what SigilFey stated above, and you wanna disable your Antivirus as well, for testing purposes.
Also what drivers do you using?
UAC is off, and i'm always using the only admin account. setting the witcher3.exe to "run this program as an administrator" and disabling my anti-virus didn't help (i believe the later was one of the things i had already tried before, but didn't remember when creating this thread).
i don't think there's an option to specify running the setup as admin since i've installed via galaxy. maybe if i download and install it manually? i'll try that.
currently installed driver is from catalyst software suite 15.7.1, which should be the latest. but i had also tried 15.7 and 14.12, doing a clean uninstall with DDU before each of them.

had some trouble with the clean boot, apparently my antivirus didn't even want to allow me to alter the msconfig settings <_<
but even after completely uninstalling the antivirus, doing the clean boot (and checking if all the settings were as shown in the link you provided, and no other programs were running on windows startup) the game still crashes as before.
about the manual download i mentioned, takes a while to download everything, but i'll try running the manual setup as admin once i've got all the files. somehow i doubt that will fix the problem, but might as well give it a try.

Alright...
Try the following:
1.) Ensure the game is installed on the root drive of your HDD/SSD. For example: C:\The Witcher 3 not C:\Program Files(x86)\The Witcher 3 (Generally, just be sure that you are not installing to Program Files or Program Files(x86). You can simply move the directory, you don't need to re-install.
2.) Add witcher3.exe and any .DLL files you find in the same folder to the exceptions list of your antivirus. I know that this is not to many people's liking, but I recommend downloading a very non-intrusive antivirus (like Avast or AVG , which are awesome and free) and setting them up in msconfig and services.msc so that you must manually activate them. Antivirus can wreak havoc on gaming (especially indie games that are not digitally signed.)
3.) Create an Elevated Administrator Account. Follow these steps. (I don't think this is it, but since you've disabled UAC, anyhoo, can't hurt!)

i tried reinstalling directly on the root drive, with the manual setup, running both the setup and then the game as administrator, using the elevated admin account. no success
i couldn't find an exceptions list anywhere in the settings of my antivirus (i'm using avira atm). but neither disabling the "real time protection" nor uninstalling it entirely made any difference. but i was thinking about trying another antivirus anyway. i've never had any issues regarding antivirus and games so far (at least not that i noticed) but there are a few things that annoy me about avira.
how would i go about setting a new one up in msconfig and services.msc and what does that change?
i still have the same problem and bought the game on release. i can't talk to yennefer or go down stairs to talk to vesemir without it crashing. when i go down stairs, geralt takes a few steps and i can hear the dialogue and skip through it but when it gets to the point where the dialogue is exhausted it crashes to desktop.
specs: i7 4790k, radeon r9 290x lightning, asrock z97 extreme 6, windows 10 (i was on windows 7 when the game was released but had virtually the same problem), 8 gb ram
i have tried many fixes. i don't know if it is a specific hardware issue with some radeon cards, i think i have seen a few similar problems with nvidia cards though but i could be mistaken. i am going to try some of the more recent suggestions in this thread (administrator, moving folders, etc.) but i am not expecting them to work, but i will bookmark for further reference. i just want to bring more attention to this issue which breaks the game from the beginning. i was fortunate enough to also have the game for xbone but it would be nice to get my pc copy working.
